Insurance Information: Patients are required to provide accurate insurance information. If we do not have the correct insurance information on file you may be held responsible for the visit. Please check our insurance tab for all our accepted insurances.
Co-payments: Co-pays are due at your appointment. Each patient is responsible for knowing his or her plan’s benefits package, co-payment, co-insurance, deductible, non-covered services, and restrictions. We cannot quote coverage or benefits. If you have questions regarding coverage or benefits, please call your insurance plan.
Shipping Policy: Empire Sleep Testing will ship the testing unit directly to your house if you chose this method. It must be to your physical address, and not a PO box. All patients who agree to have the Alice Night One device sent to them must agree to the following terms/conditions –
-Patient must have paid their co-payment in full or have agreed to a payment plan BEFORE we will ship the device to your house. It is also our policy to have an active credit card on file for all patients who are having the unit shipped to them in case the unit is kept longer than allowed, or the device does not get returned to us.
-We will mail the device directly to your house for the day you chose to perform the test. Once the device is delivered, you have 72 hours to complete the test and mail it back to us. (See our chart below for specific examples) Please make sure all the parts are put back into the device, and the paperwork is filled out and returned with the unit.
-You are subject to a $50/day charge to your account for each day you are late shipping the device back. If you need more time with the unit, please call us directly at (716)250-7460 and we will be more than happy to work with you.
-If you do not return the testing unit within 30 days, Empire Sleep Testing will consider the device stolen, and you will be responsible for the full cost of the device.
